SUMMARY OF DUTIES

This part-time position will match the needs of Aging and People with Disabilities in the community with current federal, state and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long term community-based care and nursing homes. This position will also assess eligibility, redetermine needs, and adjusts benefits and services accordingly. Also, this individual will act as an advocate for clients and aging and persons with disabilities within the community.

MINIMUM QUALIFICATIONS

A Bachelor's degree in a Behavioral Science, Social Science, or a closely related field;
OR
A Bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

ESSENTIAL ATTRIBUTES

  • Experience with managing services for individuals

  • Experience interpreting and implementing policy and procedures

  • Experience preparing and maintaining written narrative documentation of actions taken

  • Experience interviewing to obtain information to evaluate the next steps to take in a process

WORKING CONDITIONS

  • This is a part-time position and will work 20 hours per week. The schedulewill be determined at the time of interview and can be subject to change depending on business needs.You will visit clients, hospitals, nursing homes, APD offices and other providers.

  • The office is open from 8am-5pm, Monday through Friday. The needs of the public will be adequately served.This position includes physical exertion such as climbing stairs, walking on uneven terrain and transporting a laptop. General office environment. Must travel to individuals' homes, hospitals, nursing homes, and other provider settings as well as other local offices. May encounter environmental hazards including unsanitary conditions.

  • May come in contact with people who present as irrational, hostile, or angry who may become assaultive verbally, physically or with a weapon. Also, will come in contact with animals, some of which may be dangerous.

  • Drive a variety of cars which may be unfamiliar, to remote places in a variety of terrain and possibly inclement weather. Exposed to a variety of contagious diseases such as staph infections, lice, MRSA, COVID-19, etc.

  • Must have valid driver's license and acceptable driving record.
