Job description

Position Overview:

The Hospital Liaison is responsible for conducting screens and assessments for newly delivered mothers and newborns to determine eligibility status for Health Beginnings Entry Agency referral services.ย  The Hospital Liaison effectively promotes program goals and communicates accurate information about the Healthy Beginnings program. This position will provide services in assigned hospital location(s) and may require scheduling flexibility in order to accommodate hospital schedules.

Here's what the job involves:

  • Conduct screenings and assessments with new mothers and babies to determine eligibility for Healthy Beginnings services.

  • Collaborate with hospital staff and Birth Registrars to complete screenings and ensure accurate information.

  • Identify client needs, risk factors, and protective factors and connect eligible families with program services and community resources.

  • Serve as a liaison between hospitals, families, supervisors, and community partners while promoting the Healthy Beginnings program.

  • Maintain accurate documentation and program data while meeting established outcomes, attending trainings, and participating in staff meetings and supervision.

Qualifications:

  • Bachelorโ€™s degree in Human Services (preferably in a field such as social work, developmental psychology, and/or early childhood education).ย Bachelorโ€™s degree in a different field with a minimum of 2 yearsโ€™ experience with children and families.

  • Minimum of two (2) yearsโ€™ experience in child welfare, primary prevention services, child development or related field.

  • Schedule includes occasional evening and weekend hours as needed

  • Must be 21 years or older

  • Bilingual English/Spanish required.

  • Valid Floridaโ€™s driverโ€™s license is required.

  • Successful completion of a background check through Clearinghouseย (learn more)
