Job description

Full-Time Administrative LPN

Hybrid in-office and field position. Office hours and field hours may vary based on need. All current field locations within 15 minutes of the office.

Hours are flexible. Some evening and weekend medication passes required.

DUTIES:

  • Create plan of care for all new individuals served
  • Conduct supervisory visits and document results
  • Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
  • Review of data tracking and documentation
  • Provide staff training and oversight
  • Coordinate medical visits for clients
  • Medication passes
  • Other duties as assigned
