Job description

CNHHS is seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients' homes. The Registered Nurse (RN) is responsible for assessing patient needs, developing individualized care plans, coordinating services, and providing skilled nursing care to patients recovering from illness, injury, surgery, or managing chronic conditions. 

The idea candidate will have strong clinical skills, excellent communication abilities, and a passion for helping patients maintain independence while achieving their healthcare goals. This position requires collaboration with physicians, therapists, caregivers, and interdisciplinary team members to ensure optimal patient outcomes.

Qualifications:
  • Current, unrestricted Registered Nurse (RN) license.
