Our vibrant campus offers Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Hospice services, creating a community where residents—and employees—thrive.

Beyond Hospice, a service of Episcopal Homes of Minnesota, is seeking a compassionate and experienced Registered Nurse – After-Hours Triage to provide exceptional end-of-life support to hospice patients and their families during evening and overnight hours.

This role is ideal for an RN who is confident working independently, has strong clinical judgment, and is passionate about helping patients and families remain comfortable and supported throughout the hospice journey.

Pay & Schedule

Triage Pay: $200 per on-call shift
Visits: Paid at the applicable hourly RN rate for any visits made during the shift

Schedule: 8:00 p.m.–8:00 a.m.

Preferred Rotation: 7 evenings/nights on, followed by 7 days off

This is an after-hours triage position providing telephone support and making in-person hospice visits when clinically necessary.

ResponsibilitiesClinical Leadership & Patient Care
  • Complete comprehensive initial and ongoing assessments of patients and families to determine hospice needs.

  • Assess patient, family, and caregiver concerns during after-hours triage calls and determine the appropriate level of intervention.

  • Provide skilled nursing care focused on comfort, symptom management, and quality of life.

  • Make in-person patient visits during the overnight shift when clinically necessary.

  • Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team.

  • Utilize assessment findings to identify changing patient and family needs and revise the plan of care as appropriate.

  • Administer medications and treatments as ordered by the Medical Director or hospice provider.

  • Complete medication reconciliation and coordinate medication orders as needed.

  • Educate patients, families, and caregivers on medications, symptom management, comfort measures, diet, self-care techniques, and disease progression.

  • Provide emotional support, education, and guidance to patients and families throughout the hospice journey.

  • Assist patients and caregivers with activities of daily living and support the patient's comfort-care goals.

  • Coordinate medical equipment, supplies, medications, and other necessary services.

  • Serve as Case Manager when assigned by the Clinical Supervisor and coordinate all aspects of care for assigned patients.

  • Complete accurate and timely clinical documentation, including changes in condition, interventions, pain and symptom management, and outcomes.

  • Supervise and support Hospice Home Health Aides and Licensed Practical Nurses as appropriate.

  • Maintain a safe environment for patients, families, and caregivers.

Interdisciplinary Team Collaboration
  • Collaborate with physicians, social workers, chaplains, therapists, hospice staff, and other members of the interdisciplinary team.

  • Communicate changes in patient condition to the Medical Director or designated provider and obtain orders as needed.

  • Coordinate with other healthcare services to ensure continuity of care.

  • Communicate effectively with patients, families, caregivers, contracted facilities, and community healthcare partners.

  • Work closely with the interdisciplinary team to support the emotional, physical, and spiritual needs of patients and families.

  • Attend and participate in interdisciplinary team meetings as indicated.

  • Fulfill accepted patient and case assignments.

  • Participate in on-call and after-hours responsibilities as assigned.

Quality & Compliance
  • Follow CMS, Medicare, and State of Minnesota regulations and guidelines for hospice care.

  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives and activities.

  • Maintain accurate clinical documentation and uphold Beyond Hospice quality and patient-care standards.

  • Follow infection control, safety, and organizational policies and procedures.

Operational & Financial Efficiency
  • Collaborate with hospice leadership and team members to optimize resources while maintaining high-quality patient care.

  • Support efficient use of hospice equipment, supplies, medications, and services.

  • Participate in efforts to improve operational efficiency, patient satisfaction, and overall hospice services.

Community & Professional Development
  • Build positive working relationships with contracted facilities and community healthcare providers.

  • Educate facility staff regarding hospice philosophy, end-of-life care, and patient needs.

  • Positively represent Beyond Hospice within the community.

  • Maintain and strengthen professional knowledge and nursing skills through continuing education, training, and in-service programs.

QualificationsRequired
  • Graduate of an accredited school of nursing.

  • Current Minnesota Registered Nurse (RN) license.

  • Excellent assessment, observation, communication, critical thinking, problem-solving, and documentation skills.

  • Ability to work independently and make sound clinical decisions during evening and overnight hours.

  • Valid driver's license.

  • Reliable vehicle in good working condition.

  • Current automobile insurance that meets state and organizational requirements.

Preferred
  • Bachelor's degree in nursing (BSN).

  • Two (2) years of acute care nursing experience in an institutional setting.

  • Two (2) years of recent hospice or home health experience.

  • Previous hospice triage or on-call nursing experience.

Physical Requirements
  • Ability to lift, push, or pull up to 50 pounds.

  • Frequent standing, walking, bending, and reaching.

  • Ability to assist patients with mobility and transfers as needed.

  • Ability to work in patients' homes, hospice settings, contracted facilities, and other community locations.

  • Comfortable working independently in emotionally sensitive patient and family situations.

  • Ability to comply with infection control and safety procedures.
