We are seeking an experienced RN Case Manager to join a dynamic Acute Care Hospital team. This role is responsible for coordinating patient care throughout the continuum, facilitating safe and timely discharges, and ensuring compliance with regulatory requirements while optimizing patient outcomes.
Minimum Qualifications

  • Active Registered Nurse (RN) License
  • Minimum 3 years of recent Acute Care Hospital Case Management experience
  • Previous travel experience required (First-time travelers will not be considered)
  • Strong experience with:
