This remote position provides the flexibility to work from home while being integrally connected to our hospice center team. You will have the opportunity to engage with professionals from various backgrounds, allowing for collaborative care that addresses both the psychological and emotional needs of patients. • Conduct thorough assessments and evaluations to accurately identify the mental health needs of hospice patients, developing tailored treatment plans that prioritize their emotional stability.
• Deliver evidence-based therapeutic interventions, utilizing modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and supportive counseling to foster resilience in patients and their families.
• Collaborate closely with case managers, psychiatrists, and other healthcare professionals to ensure a holistic and coordinated care approach for hospice patients, adjusting plans as necessary based on patient feedback and needs.
• Provide crisis intervention support to patients and their families experiencing acute distress, offering guidance and emotional support during difficult times.
• Engage in community outreach initiatives aimed at raising awareness about mental health services available within hospice care, promoting understanding while reducing any associated stigma.

This position focuses on individuals in hospice care, who may be experiencing profound psychological and emotional challenges due to terminal illnesses, grief, and end-of-life transitions. You will be responsible for providing support not just to patients, but also to their families, helping them navigate the complexities of loss and emotional pain.

• An independent and active license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
• A completed master's degree or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or clinical Psychology.
• Strong communication skills, with the ability to build trust and rapport with a diverse client base and their families.
• Exhibit cultural competence to effectively engage with clients from various backgrounds, ensuring respectful and tailored care.
• Demonstrated organizational skills to manage caseloads effectively while maintaining accurate and timely documentation.

• Previous experience working with hospice patients or in healthcare settings.
• Familiarity with grief counseling and interventions for end-of-life issues.
• Proficiency in using telehealth tools and electronic health records to facilitate remote patient care.
