Job description

Hospice Registered Nurse Case Manager

Salary: $80-89k/yr BOE

Schedule: Monday-Friday| 8:00 AM- 5:00 PMย 

Location: North/West Ohio and surrounding areas, including:ย 

Marysville/Powell/Dublin

Requirements of the RNCM role:

  • Licensed Registered Nurseย 

  • BLS Certifiedย 

  • Comfortable working in both private homes and facility settings.

  • Valid Driver's License and Auto Insurance Coverageย 

  • Reliable transportationย ย 

  • A minimum of 1 year of Clinical experienceย 

  • Hospice experience preferredย 

About the RNCM role:

  • Conduct comprehensive patient assessments, including vital signs and medical history reviewย 

  • Travel to patients' residences to conduct visitsย 

  • Develop individualized care plans in collaboration with multidisciplinary teamsย 

  • Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.

  • Perform evaluations for hospice care along with informational visits as needed.

  • Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.ย 

  • Educate patients and families on treatment plans, medication management, and health maintenance strategiesย 

  • Submit complete, accurate, and relevant clinical notes promptly.

  • Participate in IDG and case conferences

  • Performs on-call duties per policy and as needed

  • Conducts an initial and ongoing comprehensive assessment of the patients

  • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.ย 

  • Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.

  • Provide care plan to Hospice Aide and LPN as part of the IDG team.ย 

  • All other duties as assignedย
