Hospice RN

The Hospice of Greater Cleveland (partnership between McGregor & Judson) delivers compassionate, patient-centered care to individuals receiving hospice services and provides emotional support to families during the end-of-life process. Under the direction of new leadership, the Hospice RN will contribute to a culture focused on compassionate care, teamwork, operational excellence, accountability, and continuous quality improvement.

As a Hospice Nurse, you will be responsible for completing hospice and palliative care visits in the home setting (which includes the community or agency setting) as assigned. These visits may include admission, routine, recertification, discharge and/or death visits. Follows the established plan of care to provide care, treatments, and education. Demonstrates knowledge and support of the philosophy, purpose, and goals of hospice care as well as those of McGregor and its policies.

Duties and Responsibilities

  • Utilizes the nursing process to meet the hospice care needs of assigned patients. Complete initial, comprehensive and ongoing assessments of clients. Provides necessary care and treatments in accordance with the plan of care and standards of care to promote comfort and quality of life. Evaluates effectiveness of measures implemented.
  • Collaborate with patients/families regarding goals of care and plan of care. Provides education and counseling related to hospice philosophy, goals of care, disease process/progression, hands-on-care, end of life process and expectations.
  • Complete required documentation in the EMR after each visit.
  • Updates the Interdisciplinary Team after each visit.

Required Qualifications

  • Graduated from a state approved school of nursing.
  • One year of nursing experience.
  • Current active license as a registered nurse (RN).
  • Certification (CHPN) preferred.
  • Current CPR certification (Basic Life Support).
  • Must possess a valid driver's license.
  • Must have effective and efficient time management skills, strong verbal and written communication skills and ability to work with diverse groups of people.
