Description:
As an Admissions Nurse, you will be an active member of the Interdisciplinary Team for providing skilled nursing assessments on an as-needed basis consistent with Tapestry Hospice policies & procedures. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician to establish an initial plan of care while acting as a liaison between the various team members during the admission process.
Minimum Requirements:

  • Current RN License in the State of Georgia
  • 1-2 years nursing experience (Preferred)
  • 1-2 years hospice experience (other candidates may be considered with experience in skilled nursing, home health, acute care, medsurg or ICU)
  • Utmost compassion & integrity
  • Valid Georgia driver's license, insurance and reliable vehicle
  • Excellent Time Management Skills
  • Current RN License
