Hospice Registered Nurse Case Manager (RN) - Topeka, KS

Hours: 8:30 am - 5:00 PM

We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager (RN) to join our team at Ascend Hospice in Topeka, KS. As a leading provider of home care excellence, we offer a comprehensive menu of care and services delivered by a team of highly trained and compassionate professionals.

About the Role:

As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Your duties will include but are not limited to:

• Completing initial assessments of patients and families to determine hospice care needs
• Regularly re-evaluating patients' nursing needs
• Using assessment data to determine care plan needs
• Collaborating with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care
• Counseling patients and families in meeting end-of-life needs
• Initiating appropriate preventative and palliative nursing procedures
• Communicating with physicians regarding patients' needs and reporting any changes in patients' conditions
• Supervising Hospice Aides

Requirements:

• Associate's Degree in Nursing
• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills
