University Home Care & Hospice is seeking a compassionate and dedicated Hospice Registered Nurse to join our team in Livonia, MI. This role involves providing high-quality, patient-centered care to individuals facing life-limiting illnesses, ensuring comfort and dignity in their final stages of life.

Responsibilities
  • Provide comprehensive nursing care to hospice patients in their homes or care facilities, focusing on pain management and symptom control.
  • Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.
  • Monitor patient health and progress regularly, adjusting care plans as necessary.
  • Offer emotional support and education to patients and their families regarding end-of-life care.
  • Ensure accurate and timely documentation of patient care and compliance with regulatory requirements.
  • Coordinate with physicians, social workers, and other healthcare professionals to deliver holistic care.
Requirements
