Registered Nurse (RN) Care Manager

Hybrid Community-Based Role | Wayne County, Michigan

Are you an experienced Registered Nurse who wants to use your clinical knowledge beyond the bedside?

TANDEM365 is seeking an RN Care Manager, internally known as an RN Navigator, to support individuals with complex medical, behavioral health, and social needs throughout Wayne County. This role focuses primarily on care coordination, participant education, advocacy, and healthcare navigation, with limited direct clinical care.

You will meet participants in their homes, help them understand and navigate the healthcare system, coordinate with providers and community organizations, and develop person-centered plans that support their health goals and ability to remain safely at home.

What the Hybrid Schedule Looks Like

  • Approximately three face-to-face participant visits per field day
  • Remote documentation, telephone outreach, and care coordination from home
  • Meetings at the Plymouth office two to three times per month
  • Travel throughout Wayne County, with the territory shared among the nursing team
  • Mileage reimbursement beginning when you leave home

RN Care Managers generally support 35 to 50 participants, with most participants seen face-to-face every four to six weeks, depending on their individual needs.

On-Call Expectations

  • After-hours coverage approximately two to three times per month
  • Weekend coverage approximately once per quarter
  • All on-call responsibilities are completed by telephone from home
  • Telephone triage and guidance only
  • No in-person or field response required

What You Will Do

  • Conduct comprehensive in-home and telephone nursing assessments
  • Identify medical, behavioral health, medication, safety, health literacy, and social needs
  • Develop and monitor individualized, person-centered plans of care
  • Coordinate care with primary care providers, specialists, hospitals, rehabilitation facilities, hospice organizations, and community agencies
  • Help participants and families understand diagnoses, medications, treatment recommendations, and follow-up needs
  • Provide education tailored to each participant’s goals, readiness, and health literacy
  • Support participants during transitions between healthcare settings
  • Use motivational interviewing, shared decision-making, and goal-setting strategies
  • Advocate for participants and connect them with appropriate healthcare and community resources
  • Collaborate with nurses, social workers, community paramedics, medical assistants, providers, and community partners
  • Maintain timely and accurate electronic documentation
  • Provide limited direct nursing care when appropriate and when other services are unavailable
  • Participate in the shared home-based telephone triage rotation

Required Qualifications

  • Active and unrestricted Michigan RN license
  • Minimum of two years of nursing experience
