Puget Sound Home Health and Hospiceis seeking an experienced and caring full-timeHospice Registered Nurse Case Managers (RN)inKing County, Washington (one in Renton area and one in Federal Way area)!

Essential Duties

  • Perform patient assessments (initial + ongoing)

  • Develop and update care plans

  • Communicate with physicians and team

  • Triage urgent calls/on-call rotation

  • Guide on hospice philosophy/DNR/advance directives

  • Provide end-of-life emotional support

  • Mentor new staff

  • Ensure compliance with regulations

  • Coordinate with outside services (DME, pharmacy, etc.)

Requirements and Qualifications

  • Registered Nurse (RN) licensure

  • **Prior experience working in a hospice agency is strongly preferred**

  • Ability to work independently and demonstrate critical thinking

  • Current BLS certification from AHA

  • Be a licensed driver with an insured automobile in good working order
