Schedule: Monday thru Friday, 8am - 5pm

Hospice of North Alabama is seeking a licensed LPN or RN with strong clinical and communication skills to join our team as a Hospice Intake Coordinator in Cullman, AL. This is a critical role that helps patients and families transition smoothly into hospice care by coordinating the intake and admission process from start to finish.   • Verify insurance, obtain authorizations, and document all referral and patient information. 
  • Communicate with physicians, discharge planners, and other healthcare professionals to ensure continuity of care.
