As a Hospice House RN, you’ll be on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.

YOU WILL
  • Provide skilled nursing care to Hospice House patients, maintaining comfort and quality of life throughout the endoflife journey.
  • Use clinical guidelines and knowledge of endstage diseases to assess patient status, recognize changes, and support appropriate hospice care.
  • Advocate for patients’ comfort and care needs with physicians, Nurse Practitioners, and other members of the care team.
  • Collaborate as an integral member of the Interdisciplinary Team (IDT), communicating patient needs and coordinating care with clinical and support services.
  • Address the physical, emotional, and spiritual needs of patients and their families while providing compassionate endoflife care.
  • Develop, implement, and continually evaluate individualized plans of care based on each patient’s changing needs.
  • Accurately and thoroughly document patient assessments, interventions, outcomes, and changes in condition in accordance with Medicare, regulatory, and agency requirements.
  • Ensure documentation supports the patient’s General Inpatient (GIP) level of care and demonstrates measurable outcomes and continued hospice eligibility.
  • QUALIFICATIONS
  • Registered Nurse currently licensed and registered to practice in NYS.
  • Baccalaureate degree in Nursing required.
  • A minimum of two years’ experience in the delivery of nursing care in a Certified Home Hospice Agency preferred.
  • Current BLS (Basic Life Support) certification
