Job description

Job Duties

Horizon Public Health is seeking applicants for a Full-Time Registered Nurse to provide case management services out of our Alexandria office. A Case Manager provides care coordination, including developing, implementing and evaluating client plan of care, coordinating care using available resources and maintaining contact with care givers. Specifically, this position will work with clients age 65 and older.

Successful applicants must pass a criminal background check and a pre-employment drug test. Must have valid MN driver’s license and reliable, insured transportation. This is a Grade 13 MNA/AFSCME Council #65 union position.

Starting salary range is $36.03-$38.23/hour with earning potential up to $47.01/hour. Benefits include employer sponsored health and life Insurance, HSA/VEBA contribution, PTO, and PERA retirement as well as several employee voluntary benefits to include: 457 b retirement plans, long-term disability, vision insurance, dental, supplemental life insurance, critical illness, hospital care and accident plan.

Minimum Education Required

Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN with at least one year of home and community-based experience. Exceptions may apply for ADN applicants with at least two years of home and community-based experience. Prior experience in long term care is also beneficial.
