Job description

Palliative Nurse Practitioner (NP) Position

HopeHealth is seeking an experienced Palliative Nurse Practitioner for its affiliate HopeHealth Hospice & Palliative Care.

The Palliative Care Nurse Practitioner works in collaboration with the referring provider and appropriate consultants to provide relief of suffering for patients and families facing serious illness. They will provide assessment and recommendations to relieve burdensome symptoms like pain, dyspnea, anxiety, fatigue. They will be able to communicate serious news and help patients and families establish goals compassionately and clearly for care.

The palliative Nurse Practitioner needs to be highly collaborative, working cooperatively with patients, families, referring entities, serving as an expert in hospice and palliative care. This position will be covering Newport County Hospital in Rhode Island.

This is a full time 40 hour per week position working 8am-4:30pm.

Duties / Responsibilities
  • New consults and follow up consults for seriously ill adults in a variety of inpatient and outpatient settings including but not limited to homes, assisted livings, nursing facility, hospitals, clinics.
  • Complete pain assessments and pain management recommendations for seriously ill adults.
  • Assesses and makes recommendations to manage other non-pain symptoms including nausea, fatigue, dyspnea, anxiety.
  • Assesses patient's disease trajectory and co-morbidities and, in conjunction with treating physicians, formulates prognosis
  • Works as a seasoned consultant, fostering positive and collaborative relationships with referring medical providers or entities
  • Based on prognosis and formally elicited patient and family goals, makes recommendations regarding both further treatment and end of life options
  • Counseling and Coordination of care with patients, families, providers, facilities as Improves clinical outcomes by conducting research or applying the research findings of others.
Professional Policies
  • Demonstrates knowledge of HHHPC policies and procedures and pertinent State and Federal Rules and Regulations.
  • Provides accurate and appropriate documentation and timely submission of billing
  • Consistently apply the HHHPC values in all aspects of work, complying with established HHHPC mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance.
  • Serves as "expert" in hospice and palliative medicine, providing informal and formal education, serving as preceptor to student or new hires
Education / Experience
  • Graduated from an accredited master's degree program in Nursing and/or Clinical Doctorate program in nursing.
  • Certified as a Family/Geriatric or Adult Nurse Practitioner and certified as a Registered Nurse Practitioner in the state of RI required.
  • Minimum of one to three years' experience as nurse practitioner or physician's assistant, preferably in the field of Palliative Care. Minimum of 2 years' experience in geriatrics, acute care, hospice, or oncology.
  • Experience with pain and symptom management is highly preferred.
  • Knowledge of third-party reimbursement procedures and requirements is helpful.
  • Experience in hospice and palliative care is highly preferred and with certification as advance practice in hospice and palliative care. Experience in primary care within a hospital or nursing home preferred.
  • Current RI and MA licenses as RN and Registered Nurse Practitioner or Physician's Assistant.
  • Current RI and MA licenses for prescriptive authority, RI and MA controlled substance license, DEA license.
  • Valid driver's license with and automobile that is insured in accordance with state and/or agency requirements and is in good working order.
