Job description

Position Overview
Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Team Coordinator to join our dynamic home health care team. The Team Coordinator the clinical operations and management staff members.
Hours: Monday - Friday 8:00am - 5:00pm onsite
Essential Job Functions:
  • Greet and assist clients, employees, and general public when entering office providing excellent client and employee customer service
  • Provide daily analytical support and validation through our EMR system (Homecare Homebase) per industry regulations
  • Daily activities can include but are not limited to:
    • Scheduling clinicians, notify physicians of care changes, perform pre-bill auditing
    • Send patient instructions
    • Verify receipt of proper documentation and follow up as necessary with clinicians, clients and families
    • Manage office calls, correspondance and meetings
    • Monitor office supply levels
    • Provide customer support
  • Support ongoing client and employee requests including staff scheduling, preparing client materials, administrative tasks and processing of client files in alignment of government/health care standards
  • Work collaboratively with all branches and home office to ensure seamless coordination and service amongst our business and clinical delivery team
  • Participate in staff meetings and meets regularly with branch management for individual supervision to meet performance objectives
  • Comply with HIPAA and Data Privacy Practices and regulation

Requirements:
  • Proficient with Microsoft Suite
  • Customer service experience
  • Excellent written and verbal communication skills

Preferred:
  • Prior scheduling Home Health scheduling experience
  • Prior scheduling LTC experience
  • Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
