Job description

Medical Group that has been serving the local area for over 30 years and has a network of over 200+ physicians is seeking to add a Primary Care Physician Assistant to its team.

The position is in a well established practice. As an employed PA, you will have full benefits, including Medical, Dental, Vision, Life, and a very generous 401k offering.

PAY: $100,000 to $140,000/per year DOE

  • Conduct Annual Wellness Exams (AWVs) in accordance with CMS guidelines
  • Identify and accurately document chronic conditions for HCC risk adjustment
  • Perform thorough medication reconciliation and patient education
  • Collaborate with primary care providers and care coordinators to ensure continuity of care
  • Utilize EMR systems to document findings and support coding accuracy
  • Participate in quality improvement initiatives and compliance audits
Required Skills:
  • Minimum 1 year of clinical experience (HCC or primary care experience preferred)
  • Strong knowledge of Medicare Advantage, HCC coding, and risk adjustment
  • Proficient in EMR documentation and medical coding best practices
  • Excellent communication and patient engagement skills
Preferred Qualifications:
  • CRC (Certified Risk Adjustment Coder) certification
  • Bilingual (Spanish/English)
  • Experience with in-home assessments or mobile care, willing to travel to local clinic sites within the Victorville, Apple Valley, Phelan, Hesperia.
