We’re  seeking an experienced Home Health Scheduler with true mastery of Medicare episode management. Someone who can think ahead, anticipate needs, and take ownership of the scheduling process from start to finish. This is a high-visibility position that plays a pivotal role in our daily operations and directly impacts the quality of care we provide.

What You’ll Do


  • Manage and maintain all patient and staff schedules with precision, ensuring full compliance with Plan of Care frequencies and Medicare guidelines
  • Proactively coordinate with clinicians, patients, and families to prevent visit lapses and maintain continuity of care
  • Initiate and track authorizations, monitor expirations, and renew as necessary
  • Collaborate closely with the Director of Nursing and Intake/Admissions on utilization review and LUPA prevention
  • Troubleshoot scheduling challenges with strategic thinking and problem-solving
  • Communicate clearly and consistently with clinicians, patients, and leadership
  • Cross-train in Intake/Admissions and Human Resources to provide operational support as needed
  • Maintain organized, compliant, and timely documentation

What We’re Looking For


  • Absolute proficiency in Medicare home health episode scheduling — from intake through recertification (required)
  • Minimum 1 year experience in a Medicare-certified home health agency (required)
  • Strong understanding of frequencies, authorizations, and utilization management
  • Proficiency with WellSky/Kinnser EMR (preferred)
  • Excellent communication, time management, and multitasking skills
  • A proactive, solution-oriented mindset — a thinker, not just a doer 
  • Ability to thrive in a fast-paced, high-accountability environment
  • High school diploma or equivalent (required)
