Calling All CHHA Certified Home Health RN Case Manager Registered Nurses For Rewarding Case management Position available in Rochester, NY!

RN Registered Nurse Case Management Position Description:

  • Develops care plans appropriate to the patients/families health care needs in conjunction
  • with patient, family, physician, clinical operations team and third party payers including
  • obtaining medical orders and approval of medical services.
  • Completes the OASIS Assessment as needed to accurately reflect the clinical, functional
  • and social needs of the patient.
  • Assures the completion of all appropriate clinical records needed for compliance with state
  • and federal legislation and the delivery of seamless service within established time frames.
  • Refers to appropriate Health Care Systems, URMHC and community agencies/resources.
  • Communicates to the health care team members the outcome of the assessment/
  • reassessment (if applicable) visit to ensure appropriate follow up occurs based on the
  • needs of the patient/family’s plan of care.
  • 8. Displays an exemplary level of patience, courtesy, and flexibility. Interacts with patients,
  • physicians, and URMHC staff in a manner conducive to maintain positive relationships
  • and to meet the goals and objectives of the agency.
  • Ensures problem resolution at the point of contact.
  • Maintains appropriate productivity requirements.
  • Adheres to Professional Home Visit Performance Standards.
