Job description

WELLBOUND Certified Home Health Agency is actively seeking compassionate and reliable Certified Occupational Therapy Assistants (COTAs) to provide in-home therapy services to pediatric and adult clients with special needs. Services are delivered throughout all five boroughs of New York City and Long Island. This role offers the opportunity to support functional independence and quality of life through hands-on, patient-centered care in a home-based setting.


Location: All 5 boroughs and Long Island

Compensation: $50.00–$75.00 per visit

Job Type: Fee-for-Service (FFS)


Responsibilities

• Support clients in developing fine motor, gross motor, and visual-motor skills in accordance with the established Plan of Care.

• Assist with sensory and emotional regulation, cognitive skills, and activities of daily living (ADLs).

• Implement therapeutic interventions as directed by the evaluating Occupational Therapist and within scope of practice.

• Communicate effectively and in a timely manner with patients, caregivers, and interdisciplinary team members.

• Educate and train caregivers (including Home Health Aides) to promote skill carryover and continuity of care.

• Complete clinical documentation accurately, professionally, and within required timeframes.

• Travel to multiple patient locations within the assigned service area.

Requirements

• Valid New York State Certified Occupational Therapy Assistant (COTA) License (required).

• Strong knowledge of anatomy, physiology, and sensory processing to effectively treat diverse populations.

• Experience in outpatient clinics, skilled nursing facilities, or home health settings preferred.

• Excellent communication and interpersonal skills to collaborate with patients, families, and healthcare teams.

• Ability to build rapport with patients while demonstrating empathy, patience, and professionalism.

• Strong organizational skills to manage multiple cases while maintaining high-quality care
