Salary Range: $36.00-$42.00 per hour
Home Health patient locations: Roseville, California and surrounding areas

Essentials Duties & Responsibilities:
  • Assist a Nurse Case Manager in providing direct patient care.
  • Initiate communication with team members, HHAs, and other healthcare providers to facilitate optimal care.
  • Instruct primary caregivers, patients, volunteers, and employed caregivers in care provision based on the Plan of Care as requested by the Nurse Case Manager or Team Leader.
  • Perform nursing procedures within the scope of a Licensed Vocational Nurse.
  • Maintain current documentation in a computerized clinical documentation system according to organization policies, procedures, and applicable laws and regulations.

Required Qualifications:
  • Graduate of an approved and certified LVN program.
  • Current and unencumbered license to practice as an LVN in the state.
  • Ability to make quick judgments and work independently.
  • Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines.
  • At least one year of LVN clinical experience; home health experience preferred.
  • Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication.

Skills & Attributes:
  • Strong critical thinking skills when working independently in the field.
  • Excellent interpersonal skills to build rapport with diverse patient populations.
  • High level of organization and time management to navigate a daily route.

Nice to have:
  • Experience in acute care and working in a home environment.
  • Wound care experience in wound care/vac, post-surgical care, catheterization.
  • Experience with KanTime EMR.
