Job description

Admission Care Liaison (Registered Nurse) - Part time

Location; Newark, Ohio

Schedule: Part-Time | 10:00 AM – 10:00 PM | Includes every third weekend and rotating holiday coverage.

We are seeking a dedicated Registered Nurse to support hospice admissions and care coordination for patients transitioning to end-of-life services. This role involves conducting evaluations, collaborating with healthcare providers, educating patients and families, and ensuring a smooth admission experience across hospital, facility, and home-based settings. The ideal candidate demonstrates strong clinical judgment, compassionate communication, and a commitment to quality patient care.

Key Responsibilities

• Conduct comprehensive patient and family assessments, addressing physical, emotional, psychosocial, and spiritual needs in hospitals, long-term care facilities, assisted living communities, and home settings.

• Educate patients and families about hospice services and eligibility, supporting informed decision-making and smooth admissions processes.

• Collaborate with interdisciplinary team members to create and initiate individualized plans of care that align with patient goals and needs.

• Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.

• Provide ongoing assessments, symptom management, and care coordination for patients in contracted facilities while facilitating transitions between care settings as needed.

• Partner with physicians, hospital personnel, and care teams to ensure continuity of care and timely discharge planning consistent with hospice principles.

• Support organizational operations and patient care initiatives by assisting across departments when needed.

• Participate in quality improvement activities to enhance patient outcomes and service excellence.

• Mentor and orient newly hired staff, fostering teamwork, professional development, and positive working relationships.

Qualifications:

  • Graduate of an accredited nursing school
  • Registered Nurse (RN) license in the state of Ohio without any board actions
  • Two years acute care nursing experience preferred
  • Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Valid Ohio state driver's license and acceptable driving history.
  • Ability to travel as needed in varying weather and road conditions.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to successfully complete a background investigation and 10-panel drug screening.
