Qualifications:
  • Must have NYS certification for PCA & HHA's
  • Must be 18 years old
  • High school diploma or equivalent
  • Preferred six (6) months experience
  • Meets health state requirements

Responsibilities
By following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following:
  • Monitoring patient condition by observing physical and mental condition, intake and output and reporting changes to nursing supervisors.
  • Maintaining a healthful, safe environment.
  • Providing nutritional and environmental support.
  • Assisting with personal hygiene, toileting, dressing, walking and feeding the client.
  • Positioning the client as per care plan.
  • Assists patients by providing personal services, such as bathing, dressing and grooming
  • Supports patient by providing housekeeping and laundry services, food shopping and running necessary errands as per care plan.
  • Preparing and serving meals and snacks as directed by care plan
  • Assisting with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor.
  • Reminding the client to take medications
  • Assist patient with shopping efficiently utilizing patient's money and observing cost-saving techniques.
  • Obtaining patients vitals, understanding values not within normal limits, documenting them accordingly and informing the nursing supervisor as required.
  • Utilizing Electronic Visit Verification for daily attendance and completion of timesheet
  • Maintains a safe and secure environment by following infection control protocol
  • Following dietary requirements and nutrition standards as per care plan.
  • The correct use of medical supplies and equipment when appropriate.
  • Emotional support and the development of a trusting relationship between the aide and the client.
  • Encouragement of client independence.
  • Protects the agency by following professional standards, home care policies and procedures, federal, state and local requirements.
