Job description

Home Based Primary Care Nurse

Completes independent home visits to Veterans with complex medical and psychosocial conditions. Provides direct nursing care and education to patients and caregivers based on the individualized plan of care initiated by the RN Case Manager.

Collaborates with the HBPC Interdisciplinary Team which includes the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider.

Collaborates with services internal and external to the VA to manage the care needs of the patients. Provides patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals.

Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion and health prevention.

Implements provider's orders in a timely and complete manner.

Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.

Demonstrates the ability to prioritize the sequence of assigned patients based on patients' needs.
