Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing physical capabilities and creating tailored treatment plans.
• Implement and adjust treatment programs across various specialties, including orthopedic, pediatric, neurologic, and sports medicine.
• Provide skilled therapeutic care in the patient's home, employing established procedures and techniques that align with the physician's plan of care.
• Deliver exceptional customer service and maintain effective communication with patients, caregivers, and interdisciplinary teams to foster patient engagement and satisfaction.
• Consult and educate patients as well as their caregivers on exercises and techniques to facilitate recovery, ensuring a clear understanding of their care plans.
• Meet minimum utilization goals and actively contribute to business objectives while advocating for patient needs.

Patient Population / Client Focus

You will interact with a diverse range of patients, each with unique needs stemming from various injuries and health conditions. This role allows for the opportunity to make a significant impact on individuals recovering from surgical procedures, sports injuries, age-related ailments, and more. Your expertise will not only help them regain their mobility but also empower them to manage their health proactively.

Qualifications

• A Bachelor's degree in Physical Therapy from an APTA or CAPTE-accredited program.
• Current State License in Physical Therapy.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Exceptional verbal and written communication skills, with the ability to convey complex medical concepts simply and understandably.
• Proficiency in computer skills, including word processing and patient management systems.
• A proven ability to build and maintain strong relationships with patients and their families is essential.
• A positive attitude and a genuine desire to connect with and support patients on their healing journey.

Preferred Experience

• Prior experience in home care or community-based physical therapy is highly desirable.
• Experience in diverse specialties, including rehabilitation for orthopedic, pediatric, and neurological conditions, is a plus.
• Knowledge of contemporary best practices in physical therapy and rehabilitation techniques.

Compensation and Benefits
