This role is primarily onsite, as you will be conducting home care visits to deliver you hands-on therapy services directly in our patients' living environments. The setting fosters personal interaction and deeper connections with those you serve, allowing you to witness the tangible impacts of your work in real-time. Your schedule will typically run Monday to Friday from 9 am to 5 pm, providing a balanced work-life dynamic within a supportive atmosphere.

• Administer thorough evaluations of patients' physical abilities to identify their unique rehabilitation needs, goals, and preferences.
• Design and implement personalized treatment plans that may include therapeutic exercises, manual therapy techniques, and appropriate modalities tailored to the geriatric population.
• Monitor patient progress meticulously, adjusting treatment protocols as necessary to enhance outcomes and facilitate continuous improvement.
• Collaborate with interdisciplinary healthcare teams, including medical doctors, occupational therapists, and speech therapists, to deliver a comprehensive and cohesive patient care experience.
• Educate patients and their families about therapeutic approaches, home exercise routines, and preventative strategies to maintain health and avoid injury.
• Implement safety measures and fall prevention programs that increase patient safety and comfort within their homes.
• Engage in continuous professional development by staying updated with the latest advancements in physical therapy practices and participating in ongoing training sessions.
• Adhere to all ethical and professional standards, ensuring confidentiality and respect in all patient interactions.

Your primary focus will be working with geriatric patients, many of whom are living with chronic conditions or recovering from surgery. You will play an integral role in helping these individuals regain mobility, strength, and confidence in their everyday activities. Our approach stresses the importance of understanding their histories, preferences, and unique challenges, enabling you to create deeply personalized care plans.

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and active state licensure to practice as a Physical Therapist.
• Strong interpersonal skills that facilitate effective communication with patients, their families, and other healthcare professionals.
• Proficiency in electronic medical records (EMR) and relevant documentation systems for efficient patient management.
• Knowledge of evidence-based practices in physical therapy and the capacity to apply these methods in clinical settings.
• Dedication to enhancing patient independence, quality of life, and overall wellness through tailored therapy strategies.
• Ability to function both independently and as part of a multidisciplinary team to support patient care initiatives.

• Experience working in geriatric therapy settings is preferred; however, new graduates passionate about this field are encouraged to apply.
• Familiarity with conducting home health assessments and designing home-based therapy programs would be an advantage.

The annual salary for this position ranges from $100,000 to $120,000, commensurate with experience and qualifications.
