This position is primarily onsite and involves visiting patients in the comfort of their own homes. You will experience the unique environment of home care rehabilitation, allowing a more individualized and compassionate approach to patient care. By working directly within patients' living spaces, you'll not only assess their physical capabilities but also their environment, leading to more tailored and effective treatment plans.

• Conduct thorough evaluations and diagnoses of patients, utilizing standardized assessment tools and personal observations to craft personalized treatment plans.
• Implement a diverse range of therapeutic interventions, such as manual therapy, therapeutic exercise, and patient education on pain management techniques, all grounded in best practices.
• Collaborate with a multidisciplinary team, which may include physicians, nurses, occupational therapists, and speech therapists, to ensure comprehensive care for each patient.
• Continuously monitor and document patient progress using EMR systems, adjusting treatment plans as necessary to meet evolving patient needs.
• Provide education and support to patients and their families on therapy techniques, lifestyle modifications, and preventive measures to promote long-term health and injury avoidance.
• Maintain compliance with all healthcare regulations, organizational policies, and ethical standards in delivering physical therapy services.

Your role will focus primarily on adult and geriatric populations dealing with a variety of musculoskeletal and neurological conditions. Many patients will require rehabilitation following surgery, injury, or chronic pain issues. The unique setting allows for tailored interventions that address each patient's specific challenges, environmental factors, and healing goals.

• Doctorate (DPT) or Master’s Degree in Physical Therapy from an accredited program is required.
• A valid Physical Therapist license in the state of Florida or eligibility to obtain.
• Current American Heart Association CPR/BLS certification preferred.
• Strong interpersonal skills to foster a positive therapist-patient relationship, alongside effective clinical reasoning and time-management abilities.
• A commitment to ongoing professional development and a passion for patient-centered care.

• A minimum of 2 years of experience in physical therapy, ideally engaging in home care settings, is preferred but recent graduates with passion and dedication are encouraged to apply.
• Experience using electronic medical record (EMR) systems for documentation and progress tracking.

• Competitive base salary ranging from $85,000 to $100,000 per year, commensurate with experience.
