Description Physical Therapist - Hospital at Home Program

Activa Home Health is seeking a clinically strong Physical Therapist (PT) to support our Hospital at Home Program. In this role, you will deliver hospital-level physical therapy services in patients' homes, with a focus on mobility, balance, fall prevention, and discharge readiness.

What You'll Be Doing
  • Provide skilled physical therapy services to patients receiving hospital-level care at home
  • Perform comprehensive PT evaluations addressing:
  • Strength, balance, endurance, and functional mobility
  • Gait deviations and fall risk
  • Transfers, bed mobility, and stair negotiation
  • Activity tolerance and cardiopulmonary response
  • Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness
  • Address functional decline related to:
  • Recent hospitalization
  • Acute illness or injury
  • Deconditioning and mobility loss
  • Educate patients and caregivers on:
  • Safe mobility techniques and transfer strategies
  • Fall prevention and home safety
  • Use of assistive devices and mobility equipment
  • Monitor patient progress, adjust plans of care, and communicate updates promptly with the care team
  • Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions
  • Complete timely and accurate documentation in compliance with Medicare, Hospital at Home, and agency standards

What You Bring to the Table
  • Active Physical Therapist (PT) license in the state of Florida
  • Degree from an accredited Physical Therapy program (DPT preferred)
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Strong clinical reasoning, communication, and time-management skills


Bonus Points If You Have

Experience in:

  • Acute care or hospital-based physical therapy
  • Hospital at Home or transitional care programs
  • Home health physical therapy

Skills in:

  • Gait training and balance rehabilitation
  • Fall risk assessment and prevention
  • Functional mobility and discharge planning
  • Post-acute and management of medically complex patients
  • Comfortable practicing independently in a home-based acute care setting
  • Familiarity with EMR documentation systems
