Elite Home Solution Physical Therapy is actively seeking a dedicated and knowledgeable Home-Based Physical Therapist to join our team in Highland Park, New Jersey. This full-time position offers the opportunity to work directly with patients in their homes, delivering customized physical therapy treatments that promote recovery and enhance overall mobility. The successful candidate will benefit from a competitive salary between USD 100,000 and 120,000 per year, alongside a comprehensive benefits package.

This role is entirely onsite as our therapists will be working in patients' homes across Highland Park and surrounding areas. This unique setting allows our Physical Therapists to create a comfortable and familiar environment for patients, which can significantly enhance their recovery process. You will collaborate closely with a diverse team of healthcare professionals and caregivers, exchanging insights and strategies to optimize patient care.

• Conduct comprehensive evaluations of patients to assess their physical therapy needs, developing tailored treatment plans based on individual conditions and goals.
• Utilize a variety of therapeutic techniques including exercise therapy, modalities for pain relief, and manual therapy to facilitate recovery and improve function.
• Continuously monitor patient progress, making necessary adjustments to treatment plans based on documented responses and evaluations.
• Educate patients and their families about the treatment process, exercises, and proper use of assistive devices to ensure understanding and compliance.
• Track and document all aspects of patient treatment, outcomes, and response to therapies to maintain accurate records and facilitate communication with other providers.
• Maintain current knowledge and skills by participating in workshops, professional networks, and continuing education opportunities.

Our Physical Therapists cater to a diverse patient population, including children and adults who are recovering from surgery, injury, or experiencing chronic health conditions. We emphasize holistic care, acknowledging each patient's unique circumstances and designing individualized strategies to support their health and mobility objectives.

• Bachelor’s Degree in Physical Therapy from an accredited institution.
• Current state license to practice as a Physical Therapist in New Jersey.
• Proficient communication skills, both written and verbal, to effectively interact with patients and their families.
• Basic computer skills, including knowledge of electronic health record systems.
• A highly adaptable and flexible approach to address diverse patient needs and environments.
• Current CPR and BLS certification.

• A minimum of two years of experience as a Physical Therapist.
• Experience in a home care setting or familiarity with home-based therapy approaches is highly desirable.
• Ongoing education or specialization in areas such as pediatrics, geriatrics, or orthopedics is a plus.
