Our mission is to empower seniors to age-in-place with dignity and provide their families peace of mind. We are building upon a proven Home and community based services model to create a culturally-competent and scalable PACE program. We are also building a comprehensive operating system focused on data and workflows that span across systems, processes, people, and care contexts. We want to empower our clinicians and staff with tools that deliver relevant data at the time and site of care and enable them to deliver exceptional care to our participants, which improve clinical outcomes, participant & provider satisfaction, and ultimately our strength as an organization.
We are a mission-driven, multidisciplinary team with deep healthcare, technology, and operations expertise, each inspired by our own personal stories of caring for seniors in our lives. Our name, Seen Health, was chosen to reflect our commitment to provide the highest standard of care to underserved older adults while respecting and incorporating their individual beliefs, heritage, and values, so that they can truly be seen.

  • Deliver modality-based treatments-including ultrasound, electrical stimulation (e-stim), and thermotherapy (hot/cold treatments)-in accordance with established protocols and safety standards.
  • Observe and monitor participant performance during therapy sessions; document progress and promptly report any changes in functional status, health condition, or safety concerns to the supervising PT.

Participant Engagement & Home-Based Care
  • Conduct scheduled home visits for treatments, mobility assessments, and check-ins, ensuring continuity of care for participants in their residence or alternative care settings.
  • Provide training and education to participants and caregivers to support safe mobility, fall prevention, and adherence to therapeutic home exercise programs.

Equipment & Environment Management
  • Assist with the maintenance, cleaning, and organization of therapy equipment to ensure a safe and effective treatment environment.
  • Identify equipment needs, repairs, or replacement considerations and communicate them in a timely manner.

Quality Improvement & Administrative Support
  • Participate in quality improvement (QI) initiatives aimed at enhancing care delivery, efficiency, and participant outcomes within the PACE program.
  • Support administrative tasks such as documentation, scheduling coordination, tracking participant progress, and maintaining compliance with organizational and regulatory standards.
Qualifications
  • Current Physical Therapy Assistant (PTA) certification/licensure in the state of California.
  • Experience working with older adults, long-term care, or PACE populations preferred.
  • Strong communication, observation, and clinical documentation skills.
  • Ability to work autonomously within a team-based, interdisciplinary care environment.
Location
