Health Home Care Manager Job Description:
Established in 1948, the Mission of ASCEND Mental Wellness is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community.
We are looking for Health Home Care Managers to join our Care Management Team!
ASCEND offers a comprehensive benefit package including healthcare benefits, retirement plan enrollment, and paid time off including holidays, vacation, personal and sick time. Salary based on qualifications and experience.
A $1,500 sign-on bonus is available for this position.
Health Home Care Manager Education & Qualifications:
  • A bachelor's degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing, or other human services field, or
  • NYS licensure and current registration as a Registered Nurse and a bachelor's degree; OR
  • NYS Teachers Certificate for which a bachelor's degree is required; OR
  • Bachelor's level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
  • Two years' experience (a Master's degree in a related field may substitute for up to one year of experience) either: Providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse; OR Linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing, and financial services)

Health Home Care Manager Job Responsibilities include, but not limited to:
  • Collaborate with individuals, their families, and providers to develop person centered recovery plans
  • Coordinate access to treatment and community support services
  • Advocate to eliminate any barriers to individualized treatment goals
  • Maintain electronic health records and progress notes
  • Provide direct community support to individuals, maintaining effective collaboration for the advancement of recovery goals
  • Participate in team meetings as well as networking with area providers
