Job Summary and Responsibilities
As our Home Health Nurse, you will be a pivotal healthcare hero, bridging hospital care with the comfort of home, empowering patients to heal in their own environment.
Every day, you will deliver personalized, high-acuity nursing care for a diverse range of conditions and ages directly in patients' homes. From complex wound care and infusions to vital patient education and meticulous documentation, you'll manage your dynamic schedule to provide comprehensive, life-changing support.
To thrive in this critical role, you will excel as an independent practitioner, leveraging your organizational prowess, technological fluency, and deep compassion to foster unparalleled patient engagement and serve as their best defense against declining health.
  • Completes an initial assessment of client and family to determine home health needs. Provides a complete physical assessment and history of current and previous illness. Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the client and family in meeting nursing and related needs.

*This position is eligible for a sign on bonus*
Job Requirements
Minimum Qualifications:
  • Registered nurse with current licensure to practice professional nursing in Minnesota and North Dakota
  • Graduate of National League for Nursing accredited school of nursing
  • Maintains a current CPR certification
  • Current valid driver's license
  • Auto insurance

Preferred Qualifications:
  • Minimum of two years experience, at least one of which is in the area of public health or home health nursing
