Job description

Under the direction of the Owner, Executive Director, Program Director; Assessment Program
Manager is responsible for conducting ILS/AST/SLS/CIEP/SSI assessments with the guidance of
Regional Center. Assessment Writer is responsible for observing and interaction with client to
provide an appropriate treatment plan. Assessment Writer is also responsible for establishing
timelines for realistic growth opportunities in the areas of behavior management, communication
skills, community use, home living skills, leisure exploration, self-care skills, self-direction,
social skills, and vocational skills.ย Hours are not guaranteed and based off client needs.ย 


Job Duties and Functions:
โ€ข Schedule meeting / assessment with client and/or family within 24 hours of receiving new referral
โ€ข Mailing letter and conducting house visit after 24 hours of not being able to contact consumer / family
โ€ข Interact with and observe client to identify appropriate treatment programs
โ€ข Analyze clientโ€™s needs and behaviors to identify appropriate programs
โ€ข Include baseline information in Individualized Service Plan
โ€ข Provide Individualized Service Plan to Regional Manager within a timely manner of conducting assessment, 72 hours
โ€ข Provide corrections, as needed
โ€ข Educate client and / or family of ILS/AST/SLS/CIEP services and its purpose
โ€ข Outline the goals in proposed treatment plan
โ€ข Provide appropriate intake documentation to client and/or family to acknowledge and sign

โ€ข Assessment writing experience required
โ€ข Excellent writing skills
โ€ข Great time management skills
โ€ข Experience working with individuals with disabilities
โ€ข Regional Center experience a plus
โ€ข Bachelorโ€™s Degree required in a related field; Masterโ€™s degree preferred
โ€ข Valid CA Driverโ€™s License & Valid Auto Insurance & Clear TB & Passport or BC & Social Security Card
โ€ข Pass Criminal Background Check (No Misdemeanors or Felonies)
