Job description


Primary Duties and Responsibilities
Under the direction of the Population Health Leader for Hoag Clinic, the Population Health Program Manager is responsible for identifying, implementing, and guiding projects relating to CMS, HEDIS, and other health plan initiatives. The program manager oversees the planning and the integration and provisioning of technology and systems to independent aligned to Hoag for perforamnce initiatives. The role will also serve as a liaison between Hoag Clinic and Hoag Enterprise Teams (IT, Cybersecurity, Compliance, and Marketing teams). This position is a key member of the Hoag Clinic Quality Improvement department who oversees the quality of care in order to achieve the best outcomes and patient experience.
The Population Health Program Manager displays and practices a work-style that reflects the mission, vision and values of the Hoag Medical Foundation as well as Hoag Hospital. Participates in the continuous Improvement process, maintains a working knowledge of departmental standard operating procedures, including the use of specialized instrumentation, Population Health requirements, and preventive maintenance.
Qualifications
Education and ExperienceRequired:
  • 3 to 5 years of experience with application support of physician practice systems
  • Bachelor of Science degree in Computer Science, Business, Healthcare or related field or equivalent years of work experience
Preferred:
  • Certificaiton(s) in Epic Ambulatory modules
  • Database administrator experience
  • Experience with CMS, HEDIS, Population Health and Quality Improvement programs
