We strengthen the LGBTQ+ community and individuals living with HIV through health and wellness services, supportive housing, food programs, education, and advocacy. Our work is rooted in community care, inclusion, and health equity.


Position Summary

Reporting to the Client Services Manager the Lead HIV Community Re-Engagement Specialist serves as a frontline leader responsible for conducting HIV testing services, coordinating community outreach activities, and leading efforts to identify, engage, and reconnect out-of-care people living with HIV (PLWH) to medical care and supportive services. This position combines direct client services, field-based outreach, data-driven case finding, and team leadership to support HIV prevention and treatment goals within the community.

The Lead HIV Community Re-Engagement Specialist works closely with medical providers, disease intervention specialists, community partners, and internal programs to reduce barriers to care and improve health outcomes for individuals affected by HIV.


This position reports to the Client Services Manager. This is a hourly, non-exempt position.


HIV Testing and Prevention Services

  • Conduct confidential HIV testing and counseling services in accordance with state and agency protocols.
  • Provide HIV prevention education, risk-reduction counseling, and information regarding PrEP, PEP, and other prevention strategies.
  • Facilitate referrals and linkages to HIV care, prevention services, behavioral health services, housing resources, and other supportive programs.
  • Maintain accurate testing documentation and ensure compliance with all regulatory requirements.

Community Outreach and Re-Engagement

  • Develop and implement strategic outreach initiatives to engage individuals living with HIV who are out of care.
  • Conduct field outreach in community settings, neighborhoods, events, and partner organizations.
  • Utilize outreach databases, referral networks, and community partnerships to locate and engage individuals who have fallen out of HIV medical care.
  • Collaborate with public health agencies and healthcare providers to support re-engagement efforts.

Care Navigation and Linkage

  • Assess barriers preventing clients from accessing or remaining in HIV care.
  • Initiate Medical Case Management referrals to transportation, insurance, housing, mental health, substance use, and other barriers.
  • Coordinate appointments and facilitate warm handoffs to HIV medical providers and support services.
  • Monitor client progress through successful linkage and retention milestones.

Leadership and Program Support

  • Provide leadership and mentorship to outreach and testing staff.
  • Assist with training new team members on outreach strategies, HIV testing protocols, and client engagement techniques.
  • Support outreach planning, event coordination, and community partnership development.
  • Monitor outreach activities and contribute to program performance improvement efforts.
  • Participate in program meetings and assist management with achieving grant and agency objectives.

Data Management and Reporting

  • Maintain timely and accurate documentation in agency databases and reporting systems.
  • Track outreach encounters, testing outcomes, referrals, linkage activities, and re-engagement efforts.
  • Assist with data collection and reporting requirements for grant-funded programs.
  • Review outreach trends and recommend strategies to improve program outcomes.
Requirements:

Minimum Qualifications

  • Bachelor's degree in Public Health, Social Work, Human Services, Health Education, or related field preferred. Equivalent experience may be considered.
  • Minimum of three years of experience in HIV prevention, HIV testing, outreach, case management, or related public health services.
  • Experience working with diverse populations disproportionately impacted by HIV.
  • Demonstrated experience conducting community outreach and client engagement activities.
  • Knowledge of HIV prevention, treatment, linkage-to-care practices, and community resources.
  • Valid driver's license and reliable transportation.

  • Ability to engage individuals experiencing stigma, mistrust, or barriers to care.

  • Must have reliable transportation and acceptable driving record is required.
  • This work requires the following physical activities: walking, sitting, talking in person or on the phone.
  • Occasional walking, bending, standing and driving are needed.
  • Work is performed in an office setting. Work is also performed in social and community settings, as necessary.
  • Minimal to moderate lifting is required at times.
