Job Description:
• Provide medical case management services to clients living with Human
Immune-Deficiency Virus/AIDS (HIV/AIDS) and their families utilizing the nursing
care process.
• Provides care/Treatment, linkages/referrals and medical case management to
individuals with HIV/AIDS.
• Review all clients, including those lost to care in the HAP program
• Review Viral load/CD4, Program adherence behaviors and social determinants of
health
• The staff will re-engage those lost to care, and promote strategies to return them
back to care.
• The staff will work closely with our STI partner service team to review anyone
with a co-morbidity of HIV/STI to re-engage or determine if other support is
needed.
• All data collection is cross referenced with both EMR (CareWare and EPIC) to
ensure all data collected is consistent.
• Coordinate the client's multidisciplinary care plan and communicates progress on
an ongoing basis.
• Assess, develop, implement, and monitor initial, bi-annual individualized client
care plans and Biopsychosocial.
• Serve as an advocate for clients.
• Ensure that quality control standards are met with all active and discharged
caseloads.
• Educate and demonstrate HIV health promotion, transmission, treatment models,
resources, and services to clients, families, individuals, and groups.
• Apprise supervisor of issues, developments, and changes.
• Prepares and provides oral and written reports on data related to Medical Case
Management.
• Enter client level data, progress notes, and information in our EMR (EPIC),
CAREWare, and other platforms as needed.
• Participate in Public Health Emergency Preparedness activities.
• Attend meetings and professional training.
• Performs other tasks and duties assigned, which may not be specifically listed in
the position description; however, are within the general occupational category
and responsibility level typically associated with the employee's class of work.
Required Knowledge, Skills, and Abilities:
1. Possess and maintain a valid Registered Nurse License issued by the Maryland
State Board of Nursing, or a license recognized by the Compact State
Agreement.
2. Possess and maintain a valid Cardiopulmonary Resuscitation (CPR)
Certification
3. Knowledge of computer software and platforms such as Microsoft Word, Excel,
Access, and Power Point
4. Knowledge of Epic EMR (Electronic Medical Record)
5. Ability to communicate effectively, both orally and in writing
6. Knowledge of informatics. Knowledge of health data systems, data analytics,
(gathering and analyzing data, designing workflows, measuring impact,
managing and systems.
Minimum Qualification:
At least 1 year of experience in related field
