✨ Position Overview

We are seeking a compassionate, experienced Case Manager to support clients on their journey to self-sufficiency. As a part of an interdisciplinary team, you will create individualized case plans and connect clients to essential resources including housing, mental health services, recovery programs, and employment options.


???? Key Responsibilities
  • Create and maintain a safe, supportive environment for clients.
  • Collaborate with the Lead Case Manager to address client needs around housing, finances, employment, and mental health.
  • Co-facilitate group sessions when appropriate.
  • Conduct bi-weekly, in-person case management meetings.
  • Maintain regular communication with the Lead Case Manager or Program Manager on client progress.
  • Partner with external agencies to coordinate care and treatment plans.
  • Participate in organizational meetings, team training, and events.
  • Present client psychosocial needs in interdisciplinary meetings.
  • Keep accurate, thorough case documentation and reports.
  • Elevate any incidents or grievances promptly to the Program Manager.

???? Qualifications
  • High school diploma required; Bachelor’s degree in Human Services strongly preferred.
  • Minimum 5 years' relevant experience (mental health, addiction, trauma, aging, etc.).
  • LSWA licensure required for candidates with a BSW.
  • Strong knowledge of D.C. social services and compliance standards.

  • Cultural competence and ability to work effectively with diverse populations.
  • Excellent interpersonal, problem-solving, and conflict‑resolution skills.
  • Strong organizational skills and ability to work in a fast‑paced environment.
  • Proficiency in MS Word & Excel; willingness to learn database systems (HMIS a plus).
  • Self-motivated with the ability to work independently.
