Licensed Practical Nurse — Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call: Weekend rotation
License: KY LPN license; Compact licenses accepted
Mileage: $0.43/mile
Coverage: Southern Jefferson & Bullitt Counties, KY


We’re seeking an experienced Home Health LPN to provide compassionate, high-quality care while helping patients maintain their dignity, independence, and quality of life in their homes.


Key Responsibilities

  • Provide care according to each patient’s individualized plan of care.
  • Monitor vital signs and report abnormal findings or changes in condition.
  • Administer medications, feedings, oxygen, and provide ostomy care as ordered.
  • Perform wound care, drain care, G-tube care, and patient education as needed.
  • Follow care routines established by PT, OT, Speech Therapy, and other providers.
  • Maintain infection-control standards and report potential infections or communicable diseases.
  • Accurately document all nursing services, communications, progress, and patient outcomes.
  • Ensure physician-directed patient needs are met.
  • Order medications, supplies, and feedings to maintain adequate inventory.
  • Monitor incidents, complete required reports, and notify appropriate personnel.
  • Ensure medical equipment is properly maintained and report defective equipment for repair.


Requirements

  • Active KY LPN license required; Compact accepted.
  • Home Health experience strongly preferred.
  • Wound care, drain care, G-tube, and patient education experience preferred.
  • HCHB experience preferred.
  • Strong clinical documentation, communication, and organizational skills.
  • Ability to work independently while collaborating with the patient’s care team.
