Registered Nurse (RN) Case Manager – Home Health


Location: Southeast Georgia (Johnson, Emanuel, Toombs, Montgomery, Tattnall, Evans, Candler, Jeff Davis, and Treutlen Counties)

Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call Requirement: One weekend per month (Saturday/Sunday rotation)
Mileage Reimbursement: $0.43 per mile
Productivity Requirement: Minimum of 30 points per week


Position Overview

We are seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to join our Home Health team serving multiple counties throughout Southeast Georgia. In this role, you will provide skilled nursing care, perform comprehensive patient assessments, coordinate care plans, and support patients in achieving optimal health outcomes within the comfort of their homes.


The ideal candidate will possess strong clinical skills, home health experience, and the ability to effectively manage complex patient cases while collaborating with an interdisciplinary healthcare team.


Key Responsibilities

  • Conduct comprehensive assessments of high-risk patients and identify clinical and social needs.
  • Develop, implement, and monitor individualized care plans in collaboration with physicians and healthcare providers.
  • Provide skilled nursing services, including:
    • Wound VAC management
    • PICC line care
    • Ostomy care
    • IV antibiotic administration
    • Advanced wound care
  • Coordinate care transitions and ensure continuity of care across healthcare settings.
  • Collaborate with physicians, social workers, discharge planners, and other healthcare professionals to achieve positive patient outcomes.
  • Identify and address barriers to care, including transportation, housing, and other non-medical support needs.
  • Monitor patient progress and adjust care plans as needed to ensure appropriate and cost-effective care.
  • Promote health education, disease prevention, and wellness initiatives.
  • Maintain accurate and timely clinical documentation utilizing Homecare Homebase (HCHB).
  • Advocate for patients while ensuring confidentiality, privacy, and regulatory compliance.
  • Identify opportunities to prevent adverse health events and intervene promptly when necessary.


Qualifications

  • Current, unrestricted Registered Nurse (RN) license in Georgia or Compact RN License.
  • Associate’s or Bachelor’s Degree in Nursing or related field.
  • Home Health experience required.
  • Experience with Homecare Homebase (HCHB) required.
  • Strong clinical skills, including wound VACs, PICC lines, ostomy care, IV antibiotic therapy, and wound care.
  • Case management experience preferred.
  • Case Management Certification preferred.
  • Strong assessment, critical thinking, and clinical decision-making abilities.
  • Excellent communication, organizational, and documentation skills.
  • Valid driver's license, reliable transportation, and willingness to travel throughout the assigned coverage area.
