Physical Therapist (Home Health)

Make a meaningful impact by helping patients regain independence in the comfort of their own homes. We're seeking a compassionate and experienced Home Health Physical Therapist to join our growing team. If you're passionate about delivering high-quality, patient-centered care and enjoy working independently, we'd love to hear from you!


Schedule

  • Monday – Friday
  • 8:00 AM – 5:00 PM
  • 40 hours per week guaranteed
  • No on-call requirements


Territory

Coverage includes:

  • York County
  • Cumberland County
  • Dauphin County
  • Perry County


Mileage Reimbursement: $0.62 per mile

Productivity Expectation

  • 30 productivity points per week


Required Qualifications

  • Active Pennsylvania Physical Therapist License
  • HomeCare HomeBase (HCHB) experience required
  • OASIS documentation experience required
  • Current CPR certification (if required by state regulations)
  • Valid driver's license, reliable transportation, and current auto insurance


Responsibilities

  • Perform comprehensive physical therapy evaluations based on physician orders and referrals.
  • Develop individualized treatment plans with measurable goals that promote patient independence and improved function.
  • Assess patient progress regularly and modify treatment plans as needed.
  • Review patient medical history, diagnoses, precautions, and contraindications before initiating treatment.
  • Communicate patient progress and care needs with physicians, interdisciplinary team members, patients, and caregivers.
  • Recommend and assist with adaptive equipment to improve patient mobility and independence.
  • Complete accurate and timely clinical documentation, including OASIS assessments and visit notes in HomeCare HomeBase (HCHB).
  • Ensure compliance with all company policies, HIPAA regulations, professional practice standards, and state and federal guidelines.
  • Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with Pennsylvania practice regulations.
  • Participate in staff meetings, continuing education, and ongoing professional development.
