Job description

Registered Nurse (RN) – Home Health

Location: Washington County, MD


Schedule

  • Monday–Friday | 8:00 AM – 5:00 PM
  • 40 Hours Guaranteed: Yes
  • On-Call: Rotating every 3rd weekend


License Requirements

  • Active Maryland RN license or Compact RN license


Productivity

  • 30 points per week


Mileage Reimbursement

  • $0.43 per mile

  • PICC line experience
  • Wound Vac experience preferred


Job Summary

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care, comprehensive patient assessments, and case management for patients in their homes. The ideal candidate will coordinate care with physicians and interdisciplinary teams while promoting quality outcomes and ensuring compliance with clinical standards.


Key Responsibilities

  • Perform comprehensive patient assessments and develop individualized care plans.
  • Deliver skilled nursing care and monitor patient progress.
  • Coordinate care with physicians, therapists, and other healthcare professionals.
  • Manage medications, physician orders, and clinical documentation.
  • Educate patients, caregivers, and staff on health maintenance and disease prevention.
  • Supervise LPN/LVNs as needed.
  • Maintain accurate documentation using HCHB and OASIS.
  • Participate in rotating on-call coverage and respond to patient needs as required.


Qualifications

  • Active Maryland RN license or Compact RN license.
  • Home Health experience required.
  • HCHB and OASIS experience required.
  • Experience with PICC lines required; Wound Vac experience preferred.
  • Strong assessment, communication, and case management skills.
