Heart Failure Cardiology Nurse Practitioner - Mercy Health Youngstown, Ohio
Heart Failure Cardiology APC Opportunity:
Bon Secours Mercy Health Physicians -Youngstown is seeking a motivated and capable Nurse Practitioner or Physician Assistant to provide outpatient coverage at the CHF clinics at St. Elizabeth Youngstown Hospital, St. Elizabeth Boardman Hospital and St. Joseph Warren Hospital. The heart failure program includes 3 Advanced Practice Providers, 3 heart failure nurse coordinators, 20 RN's, a community health worker and a dietician that rounds monthly. This position will rotate at each clinic throughout the week seeing outpatient post hospital CHF visits, treat acute HF exacerbations, titrate GDMT(Guideline Directed Medical Therapy), investigate heart failure etiology, assess arrythmias and cardiac device needs, as well as address and treat HF risk factors.
Cardiology experience is preferred.
