Healthcare Talent is assisting our client in hiring a Personal Care Coordinator (PCC) to work under the direction of the Supervisor of Case Management. This position will support members in completing their health risk assessment (HRA). The PCC will ensure communication of the member's HRA and care plan with the member, Primary Care Provider (PCP), and health care team. The PCC will identify barriers to member's care and assist in improving these barriers for all levels of care. He/she will work closely with the PCP and health care team to ensure member access to timely services and coordination of care.

Job Description

Position Responsibilities:

Outreach to members to coordinate completion of the initial and annual HRA. 

Conduct telephonic HRAs.

Data entry of HRAs received via mail.

Collaborate with PCP and health care team to ensure timely communication of member's clinical information.

Ensure appropriate documentation of all interventions.

Collaborate with licensed professionals in the development of an initial care plan for each member, incorporating the HRA and all assessment findings. 

Facilitate communication of initial care plan to the PCP and member, as necessary.

Maintain documentation of member's case, care plan and interdisciplinary care team within he organization's medical management system.

Facilitate referrals to behavioral health services.

Identify need for referrals to Long Term Support Services (LTSS) and community resources.

Facilitate warm transfers to member's case manager when appropriate, to meet member needs.

Guide members in understanding and accessing the benefits they are entitled to under Medi-Cal. 

Notify member's care team of key event triggers.

Maintain compliance with established departmental productivity guidelines.

Ensure reporting of productivity metrics to supervisor as required.

Other projects and duties as assigned.


Possess the Ability To:

Maintain accurate records and documentation.  

Effectively interview members to determine strengths, problems, functional status, goals and need for specific services/resources.

Contribute to the development of a plan based on assessment of the member's physical, psychosocial, functional, strengths/barriers and mental health needs.

Develop rapport and establish and maintain effective interpersonal relationships with internal and external contacts at all levels.

Work with Case Management staff to expedite the resolution of member concerns.

Remain knowledgeable of the Medi-Cal benefit set.

Communicate effectively, both verbally and in writing with individuals from varying cultural and ethnic backgrounds.

Speak, understand, read and/or write a second language in addition to English may be required for some assignments.

Qualifications

Experience & Education:

High school diploma or equivalent required. 

Bachelor's degree desired. 

2+ years experience working with the needs of seniors or persons with disabilities (SPD) in a customer/member service capacity.  

HMO, Medi-Cal and health services experience preferred. 

Bilingual in English/Spanish, English/Vietnamese, English/Korean, English/Mandarin, English/Arabic or English/Farsi is preferred.


Knowledge of:

Medi-Cal eligibility and benefits preferred.

Long term services and supports.

Issues that face members with special needs, such as seniors and persons with disabilities.

Principles and practices of health care service delivery and managed care, Medi-Cal, and the organization's eligibility and benefits.  

Principles and techniques to serve the SPD population in diverse social and ethnic groups.

Effective charting practices and guidelines.

Computers, keyboarding, and appropriate software to produce correspondence, charts, spreadsheets, and/or other information applicable to the position.
