Job description

Are you passionate about assisting senior citizens? Are you a warm conversationalist who enjoys understanding people’s needs? Do you enjoy the flexibility of setting your own schedule? Always Family Homecare is seeking Care Coordinators to join our staff. We are dedicated to helping senior citizens to navigate their options to find the best solution for their care needs. You will report directly to the owner of a growing company. Our Care Coordinators network with the community, families, and senior citizens. You will represent Always Family homecare to educate on how home care and future company services can benefit seniors in their homes and with managing their care. You will conduct in-home visits to understand their needs and explain how we can help. You will serve as the first impression of our company to new clients and partner organizations, upholding our highest standards of integrity and customer care. An ideal candidate will have prior connections with organizations who refer their patients to home care and will also enjoy networking in the community to find opportunities to provide care. This is your opportunity to find creative ways to reach seniors and help them lead fulfilling lives!

Location: In the field, including at local events and in client homes

Key Responsibilities

  • Network with partner health care providers to grow company referral sources and generate leads
  • Meet with senior citizens in their homes to understand their needs, explain how home care can make a positive difference, explain the client service agreement
  • Coordinate with agency team members as needed to facilitate care implementation Follow up with new clients to gather feedback on the intake process and how well care is meeting client expectations
  • Follow up on agency leads from other sources
  • Conduct marketing events in the community to raise awareness of services offered
  • Recommend complementary services to home care that can fill community needs
  • Maintain a detailed understanding of all care services offered by Always Family Homecare

Knowledge Requirements

  • Bachelor Degree and 2 years of related senior care experience (or Associate Degree with demonstrated experience)
  • Experience with home or home health care coordination, Assisted Living, Skilled Nursing, or related field
  • Prior sales experience may be helpful, but not required
  • Willingness and means to work on a commission basis, results based compensation
  • Demonstrated ability to quickly establish rapport with clients
  • Familiarity with ERM systems
  • Highly proficient English communication skills, verbal and written
  • Detail oriented and able to work without direct supervision
  • Comfortable with phone conversation, email, texting, and video calls
