Overview
Talin Home Health is seeking a dedicated and skilled Physical Therapist to join our team. In this role, you will be responsible for evaluating and treating patients to improve their physical function and quality of life. You will work with clients in their homes collaborating with other healthcare professionals to develop comprehensive care plans tailored to each patient's needs. Your expertise in medical terminology, anatomy, and patient care will be essential in providing effective treatment and support.
Responsibilities

  • Conduct thorough evaluations of patients’ physical conditions and functional abilities.
  • Develop individualized care plans that address the specific needs of each patient.
  • Implement treatment programs utilizing various therapeutic techniques, including exercises, manual therapy, and modalities.
  • Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Educate patients and their families on the rehabilitation process, home exercises, and injury prevention strategies.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning for inpatient cases.
  • Maintain accurate documentation of patient evaluations, treatments, and progress in accordance with clinic policies.
Experience

  • A degree in Physical Therapy from an accredited institution is required.
  • Proven experience in a clinical setting is preferred, particularly in orthopedics or home community care.
