Health coaching is the method of guiding a person in addressing health issues and the need to improve health. This helps a person to lead a healthy lifestyle by making behavioral and lifestyle changes. A health coach is a professional who provides coaching or counseling services to patients or anyone who is interested to build a healthy life. Health coach responsibilities are carried out by guiding people how to live a healthier, more balanced life using nutrition and lifestyle choice. Such a person helps to overcome health-related issues and provide support systems to overcome those obstacles.

SHIFT AND SCHEDULE

Full Time, Monday - Friday; 8:00 AM - 5:00 PM

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS

• Contacts and performs initial interviews with individuals who are eligible for health coaching programs.
• Provides necessary coaching to reduce or eliminate behaviors that are considered high-risk.
• Develops and conducts seminars and conventions for family members, clients or even employees who are interested in learning more about health and wellness.
• Creates and distributes health education materials such as brochures to clients.
• Develops care plans that are custom-tailored to individual clients.
• Coordinates with clients’ other healthcare providers in order to provide a well-rounded experience.
• Identify the need for behavioral change to improve health status, reduce health risks and improve quality of life
• Examine and record the vital signs like body temperature, blood pressure, pulse rate, weight, height, etc., accurately
• Receive and direct phone calls, schedule appointments, check-in patients, obtain necessary patient information to file and update patient records, and ensure all forms and consents are completed by patients
• Check-out patients, assist with referral processing, and arrange laboratory services
• Manage inventory of medical supplies and equip exam rooms with appropriate supplies

EDUCATION AND EXPERIENCE

• Possess at least a bachelor’s degree in a field such as health education, nursing, athletic training or nutrition.
• Excellent knowledge in health, nutrition and fitness issues
• Knowledge of health-related diseases and remedial measures
• Excellent verbal and written communication skills
• Strong motivational and convincing power
• Ability to counsel clients of all ages and problems
• Immense ability to develop a health plan

PHYSICAL REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential responsibility satisfactorily. Reasonable accommodations may be made to enable individuals
with disabilities to perform the essential functions. The individual must be able to:
    • Stand, walk, sit, stoop, reach, lift, see, speak and hear. Lifting is limited to 35 lbs. for clinical staff and to 50 lbs. for non-clinical staff. The individual must use an assisted-lift
device or get another individual(s) to assist with the lift that is over these maximum limits.
