JOB SUMMARY

Under the direction of the ECM Program Director, the RN Care Manager delivers comprehensive care management services to members with complex medical and behavioral health needs. This role is responsible for outreach, member engagement, clinical assessment, care planning, and coordination of services across internal and external providers to ensure timely, high-quality, and patient-centered care.

The RN Care Manager works collaboratively within the Community-Based Care Management Entity (CB-CME), supporting members, families, and interdisciplinary care teams to improve health outcomes and promote self-management.

Requirements:

MINIMUM & PREFERRED QUALIFICATIONS

Education/Training

Minimum: Associate Degree in Nursing (ADN)

Preferred: Bachelor of Science in Nursing (BSN)


Experience

Minimum: 3+ years of care management experience in a healthcare setting

Preferred: Experience in managed care or health plan environment. At least 1 year of clinical experience in acute care, SNF, home health, or outpatient setting

Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position.


Certification(s)

Active, unrestricted California Registered Nurse (RN) license

Maintains a valid California Driver’s License and current automobile insurance when using a personal vehicle for Company business


Skills, Knowledge & Abilities

· Proficiency in evidence-based communication (e.g. Motivational Interviewing)
