POSITION TITLE: SUBSTANCE USE COUNSELOR โ€“ HOC SUD

POSITION PURPOSE: The Substance Use Counselor โ€“ HOC SUD provides individual counseling for intake, assessment, treatment planning, crisis intervention, and discharge planning for clients with substance use disorder or co-occurring conditions. The Counselor โ€“ HOC SUD conduct comprehensive, individualized screening and assessments, facilitate educational workshops, process groups, and recovery services.

REPORTING RELATIONSHIP: The Substance Use Disorder Counselor โ€“ HOC SUD reports to Substance Use Disorder Manager.

PROGRAM SUMMARY FOR HEALING OAKS CLINIC- SUBSTANCE USE DISORDER (SUD) TREATMENT PROGRAM

The Healing Oaks Substance Use Disorder Treatment Program provides outpatient substance use disorder (SUD) treatment, recovery, and ancillary services to adults aged 18 years and above, with substance use issues, including co-occurring mental health disorders. Services includes client and family centered, individualized, clinically effective, outcomes-driven, culturally competent, gender specific, trauma informed, developmentally appropriate, and inclusive of evidence-based treatment practices and recovery service approaches. Co-occurring disorder services includes clinical assessments for substance use and mental health conditions, care coordination and a โ€œwarm handoffโ€ to appropriate level of care in the mental health system when need exceeds services available through the SUD program. Evidence based practices is utilized for all clients, including use of specific EPBs that demonstrate success for justice-involved populations as appropriate.

ESSENTIAL RESPONSIBILITIES AND DUTIES:

  • Completes comprehensive clinical assessments;
  • Provides clinical treatment and facilitates three psychoeducational SUD groups per day;
  • Provides individual substance use counseling;
  • Develops relapse prevention and discharge plan with client;
  • Delivers recovery services;
  • Provides ongoing multi-disciplinary care coordination, biopsychosocial assessments;
  • Individual and group counseling, case management, and rehabilitative support services to all clients;
  • Works with clients to develop service plans, specifying goals and objectives, types of services, and therapeutic interventions;
  • Performs crisis intervention as needed;
  • Assesses and identifies clientsโ€™ needs for supportive services on an ongoing basis, acts as a resource, performing case management services by assisting in obtaining, and/or makes appropriate referrals for necessary or requested services (e.g., housing, vocational, employment, food, etc.) and follows up with referral party to ensure client connected to services;
  • Leads psychotherapy and skills development groups;
  • Collaborates with other service team members on client concerns;
  • Attends, participates in and provides information about assigned clients in service review and care coordination meetings and consultations;
  • Requests assistance and guidance from management and professional staff when necessary to address client issues and problems;
  • Establishes and maintains contact with community resources to ensure continuity of care (e.g., case managers, conservators);
  • Creates detailed documentation of client progress using an electronic health record system, and performs various other administrative functions associated with the program;
  • Maintains client charts and care coordination databases;
  • Updates assigned clientsโ€™ progress notes;
  • Maintains daily log of clients seen;
  • Writes necessary communication (e.g., letters for court appearances);
  • Completes pre-assessments, intake assessments, updated assessments, and other necessary paperwork, including service plans, updates, and chart reviews;
  • Enters necessary data and information in the appropriate files and systems;
  • Completes all other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of professional principles, procedures and techniques used in the care and treatment of patient with psychiatric and addiction issues.ย Evidence-based practices and issues pertaining to psychosocial treatments for SUD, Motivational Interviewing, and Relapse Prevention. ASAM criteria and continuum of care, withdrawal management, MAT, Opioid treatment services; Evaluation of client intake/assessment, treatment planning, group counseling, individual counseling, family therapy, collateral services, crisis intervention, case management, discharge, recovery services;
  • Clinical assessment of clients with severe mental illness, principles and practices of clinical mental health triage and walk in service models;
  • Bio-psychosocial Rehabilitation (BPSR) guidelines, principles of care and recovery treatment modalities;
  • Evidence-based practices and issues pertaining to pregnant women, women with dependent children, women attempting to regain custody of their children, postpartum women and their children and substance exposed infants;
  • Knowledge of working with adults and families with co-occurring disorder, perinatal services for women and girls, gender responsive treatment, deaf and hard of hearing clients, adolescent services, clients involved with justice system, homeless population, LGBTQ+, veterans, and tribal communities.
  • Experience working with a culturally diverse population and knowledge of cultural issues, values, & beliefs of the target population of adults w/ SMI, homelessness, co-occurring disorders, and criminogenic risks and needs, relevant to providing appropriate SUD program services;
  • Exercise sound, expert independent judgment within general policy guidelines;
  • Communicate clearly and concisely;
  • Operate a computer using word processing, spreadsheet, and database software applications;
  • Operate standard office equipment.

EXPERIENCE AND TRAINING: A bachelorโ€™s degree or higher in the field of social work, behavioral sciences, psychology, or a related field and three (3) years of progressively responsible experience in a mental health setting; or an equivalent combination of training and experience. Evidence-based practices and issues pertaining to psychosocial treatments for SUD, Motivational Interviewing, and Relapse Prevention.

LICENSES, CERTIFICATES AND SPECIAL REQUIREMENTS: A valid and active registration and certification as a SUD Counselor by the State of California. A Valid Driverโ€™s License with current California automobile insurance; A Valid Cardiopulmonary Resuscitation (CPR) Certification; A Valid Tuberculosis Blood (TB) test.
